We recommend you visit these websites and documents to become more knowledgeable about children’s health issues as well as the latest information on immunization schedules and vaccines.
New patient forms must be filled out before your first visit. For your convenience, these forms may be downloaded and filled out before you arrive for your first schedule appointment. Forms are also available at the reception desk.
Please bring these forms with you along with your insurance card. The legal guardian of the patient MUST be present for your child’s first visit. Below is a check list of items you will need for your first time visit:
To ensure your billing file is current, please bring your insurance card to every visit. Patients with Medicaid: If your Medicaid card is showing you have contracted with a private insurance company but it is no longer active, the number to call to have it removed is (800) 783-5386.
If Angelina Pediatrics is not listed as your child’s primary care provider (PCP), the number to call to get Angelina Pediatrics designated as your child’s medical home is (877) 644-4494.
If Medicaid has your child’s name misspelled and/or wrong date of birth listed, please call 211 and follow the prompts to correctly change this information.
Without a monthly letter, how will a person know if he or she has Medicaid coverage that month?
